STEP 1: Click on the Membership Button you want.
STEP 2: Complete all form fields.
STEP 3: Click “Register” button on the bottom of page and you will be taken to the payment site to complete your transaction.

(ORGANIZATIONAL MEMBERSHIPS: The $150 fee will be charged to the Organizational Member #1 of 3 below. You will need to return to this page to enter details for ‘Organizational Member #2 of 3’ and ‘Organizational Member #3 of 3. If easier, you can send the page link to those professionals and have them enter their details since you’ll be asked to create a password.   There is no fee for #2 and #3 as they are included with the $150. There is a $45 fee for additional members beyond the initial three.)
